Ask if you have out-of-network benefits –
"Does my plan include out-of-network benefits for mental health care, specifically for outpatient psychotherapy at a psychologist’s office?"Check your deductible –
"Do I have a deductible for out-of-network mental health services? If so, how much is remaining before my health plan starts to reimburse me?"Determine reimbursement rates –
"What is the maximum amount my plan will reimburse for mental health service code 90834 with a Psychologist?"
If unclear, ask: "What is the maximum allowed amount for service code 90834, and what percentage will my plan cover?"
